All the paperwork you need for any HOSA event is right here!
MEMBERSHIP APPLICATION
This is the application that you fill out to apply for yearly membership in HOSA. The Google Form at the bottom of this page may be filled out and submitted online, or the paper copy should be turned in to Mrs. Smith in Room 412. Membership dues of $30 should be submitted to Mrs. Smith with either application.

This is a list of all of the State Leadership Conference competitive events with requirements.
HOSA VOLUNTEER HOURS LOG
Use this log to document any healthcare-related volunteer hours you complete. Fill out the log and have it signed by the person in charge at your site. Turn it in at the end of the year for participation points.
TEACHER APPROVAL FORM
This form must be completed by the student and signed by any teachers whose class will be missed due to a HOSA activity.
HOSA PARTICIPATION FORM
This form must be filled out and signed by both the student and guardian for every Georgia HOSA sponsored event.
FAYETTE COUNTY FIELD TRIP FORM
This form is required by Fayette County for any event during which we leave the school building.
OVERNIGHT FIELD TRIP FORM
This form must be filled out and signed for any field trip which involves an overnight stay.

This online application may be filled out to apply for membership. Membership enrollment will not be complete until the dues of $30 are submitted in Room 412.
